Pay fee Baht 50/copy at the Office of Financial Management (Hua Mak P. building, Bang Na SM114).
Submit Petition to Office of the Registrar, Window 7 (Hua Mak) or SM116 (Bang Na)
Receive Transcript as instructed

Assumption University doesn't recognize name changes after graduation.
Au Certification may be issued upon request acknowledging name change.
