Prepare Petition for Name/Surname Change/Correction (R.31) Fill Information and Print R.31 form by Acrobat
Attach copies of:

- Name/Surname change certificate.
- Thai Citizen ID/Passport.
- Residence Registration (Thai)

Submit the request at the Office of the Registrar, Window 7 (Hua Mak) or SM116 (Bang Na)

Apply New ID Card.

**Remark :
- Assumption University doesn't recognize name surname changes after graduation.
- Au Certification may be issued upon request acknowledging name surname change.
