Prepare Request for ID Card (R.21) Fill Information and Print R.21 form by Acrobat
Pay fee Baht 100 at the Office of Financial Management (Hua Mak P. buliding, Bangna SM114).
Submit petition and receipt to Office of the Registrar, Window 7 (Hua Mak) or SM116 (Bang Na).
Attach one 1" color photograph in Uniform and old ID Card.
Your Request will be processed within two days (working day).
